Choose Health LA Kids is a partnership between the Los Angeles County Department of Public Health and 20 funded agencies throughout LA County working towards reducing childhood obesity among children ages 0-5. Funded by First 5 LA, Choose Health LA Kids’ funded agencies are engaging the community through a variety of activities including grocery store tours, food demonstrations, parent education, and linking community resources to families with children ages 0-5.

The Community Resource Guide for Families/Caregivers of Children Ages 0-5 is a comprehensive publication focusing on Los Angeles County.

Note:  Clinic is CLOSED on SECOND, FOURTH OR FIFTH  Tuesday.

Please be advised that you need to bring in proof of household income and must fall in the 200% of the poverty line in order to be seen.  All appointments are about 4 to 6 weeks out.  If you qualify for the Affordable Care Act (ACA), then you must first apply for this insurance.  If you qualify for insurance, then you will no longer need to be seen by us.

The Clinic will ONLY serve patients with the following medical conditions:

Cardiovascular Disease
High Cholesterol

Thyroid related disease​

Please be advised that the clinic does not provide any of the following:

1. Mammogram screenings for women over age 40.  Those over over 40 with symptoms will be seen.  See
2. Pap smears with pelvic exams.  See
3. Surgical procedures
4. Immunizations
5. Physical Exams/Laboratory Work – patient will be referred to other clinics
6. STD and HIV testing (Please see  Currently, the North Hollywood Health Center is the closest  place for such testing.
