Health Homes Charrette Glendale Community Meeting

CHAR•RETTE (PRONOUNCED [SHƏˈRET]) NOUN A MEETING IN WHICH STAKEHOLDERS IN THE COUNTY MAP OUT LONG-TERM, SYSTEMIC SOLUTIONS TO HOMELESSNESS.
The Health Homes Charrette brings together stakeholders to engage communities to provide recommendations to the state Department of Health Care Services (DHCS) for implementing AB361, the Health Homes Bill.
The end result will be a workable, sustainable funding source for health home models and services that ensures health care and social services for chronically homeless individuals and frequent users of hospitals in Los Angeles.
Who Should Attend?
All community stakeholders in Glendale, Pasadena, and East Los Angeles are invited to a community dialogue hosted by Glendale Adventist Medical Center, Glendale Memorial Medical and Hospital Center, and Ascencia, in concert with CSH.
If you work for a hospital, a community clinic, a mental health provider, a substance abuse treatment provider, a homeless services provider, a housing developer, or an advocacy organization – and you work with chronically homeless individuals, join us.
